Start by creating a "Diamond Card" shape using custom shape tool.

 

Enable "Subtract from shape area" form the top bar. Create another diamond card shape.

 

 

Similarly create another frame with more width and height.

Double click the layer to open layer style window. Apply layer style with the settings shown.

 

 

 

Move this layer below all the layers in layer order in layer palette.

 

 

Select the black frame layer. Double click the layer to open layer style window. Apply layer style with the settings shown.

 

 

Select both the layers and merge. Press Ctrl+T. Right click the layer and select "Perspective". Adjust node to get the shape as shown.

Duplicate the layer and scale down. Move this layer below all the layers in layer order in layer palette.

Press Ctrl+L to open level settings. Apply settings as shown.

 

 

Similarly create another duplicate, scale down and change level settings to more darker shade.

 

Select and merge all the layers. Duplicate the layer and change the layer mode to "Linear Light" and opacity to 50%.

 

 

Create an orange to transparent to lime gradient. Create a new layer and apply in angle mode as shown.

 

 

Ctrl+Click the frame layer thumbnail to get the selection. Press Ctrl+Shift+I to inverse the selection and press delete.

 

Change the layer mode to "Linear Burn".

 

Similarly create "C" with 3d extrude look.

 

 

Create a line diagonal using line tool.

 

 

Change the layer mode to "Color Dodge".

 

 

Similarly create more lines and change their layer mode.

 

 

Type "Company name and your punch line as shown.
