Create a circle using elliptical shape tool. Make sure you press shift while creating this so that its a circle rather then an ellipse.

In layer palette double click the layer to open layer style window. Apply settings as shown.

Enable "subtract from shape area" at the top bar.

Create the circle as shown below.

Similarly create another circle using ellipse shape tool to create the other ear.

Create an ellipse using elliptical shape tool.

Create a line using Line shape tool shape tool.

Similarly create another line using Line shape tool.

Create an ellipse for eye using elliptical shape tool.

Create another ellipse for eyes ball using elliptical shape tool.

Press Ctrl+G to group the eyes layers and duplicate the group layer of eye and place it on the other side. Duplicate the layer. Press Ctrl+T. Right click and select Flip Horizontal and place on the other side.

Create an ellipse for the nose using elliptical shape tool.

In layer palette double click the layer to open layer style window. Apply settings as shown.

Create an ellipse for the cheek using elliptical shape tool.

In layer palette double click the layer to open layer style window. Apply settings as shown.

Press Ctrl+J to duplicate the layer and place it as shown.

Create an ellipse for mouth using elliptical shape tool.

In layer palette double click the layer to open layer style window. Apply settings as shown.

Using pen tool create a shape as shown below for the hair. Move this layer below all the layers in layer order in layer palette.

Similarly draw a hair band of black color.

Create an ellipse using elliptical shape tool.

Enable "subtract from shape area" at the top bar.

Create an ellipse using elliptical shape tool.

Select "arc" from preset warp shape. Apply settings as shown.

Create a rectangle using rectangular shape tool. Press Ctrl+T. Right Click and select perspective. Adjust the nodes as shown. Press Ctrl+J to duplicate the layer.

Create an ellipse using elliptical shape tool.

Enable "subtract from shape area" at the top bar. Create a rectangle using rectangular shape tool.

Create a rounded rectangle using rounded rectangle shape tool.

Press Ctrl+T. Right Click and select perspective.

Move the top handels inwards as shown.

Move this layer below the muffler layer in layer order in layer palette.

Create a rounded rectangle (radius 20 px) using rounded rectangle shape tool.

Press Ctrl+T. Right Click and select perspective.

Adjust the nodes as shown.

Enable "subtract from shape area" at the top bar.

Create an ellipse using elliptical shape tool.

Press Ctrl+T. Right Click and rotate few pixels.

Using pen tool create a shape of hand as shown below.

Select arm and hand layer and duplicate. Duplicate the layer. Press Ctrl+T. Right click and select Flip Horizontal and place on the other side.

Create a rectangle for the book using rectangular shape tool. Move this layer below the hand layer in layer order in layer palette.

Press Ctrl+T. Right Click and select perspective.

Adjust the handles as shown.

Duplicate the layer. Press Ctrl+T. Right click and select Flip Horizontal and place on the other side. and adjust as shown.

Create a rectangle using rectangular shape tool. Move this layer below the dress layer in layer order in layer palette.

Press Ctrl+J to duplicate the layer and adjust as shown below.

Using pen tool create a shape of shoes as shown below.

Press Ctrl+J to duplicate the layer and place it as shown below.
